Important Information

  • Regardless if you are using insurance, or paying out of pocket, a referral from your healthcare provider is ALWAYS required.

  • If I have not received a referral 48 hours before your appointment, your appointment may have to be cancelled/rescheduled.

  • A credit card is required to be on file to hold your appointment.

  • At least 24 hours notice is required to cancel or reschedule your appointment. Your credit card will be charged for late cancellations or no shows.

  • If using insurance: please make sure that you have verified your benefits, prior to your appointment.

Insurance

I am in-network with Medicare, Cencal & Blue Cross.

Coverage varies by plan, so it is important to verify your benefits prior to your appointment.

It is your responsibility to check your benefit coverage prior your appointment. See below for info on how to do this.

**If insurance declines to cover your visit(s) for any reason, you are responsible for the full amount billed.

If I am out-of-network with your insurance, sessions may still be covered. I can provide you with a superbill to submit to your insurance for possible reimbursement, upon request. However, payment is due from you at the time of service.

MEDICARE

Medicare ONLY covers Medical Nutrition Therapy (MNT) for the following conditions:

  • Diabetes

  • Chronic Kidney Disease (CKD) stages 3, 4, 5 (pre dialysis)

Medicare DOES NOT cover prediabetes, weight management or other diagnoses.

Medicare allows 3 hours of nutrition therapy the first year and 2 hours for subsequent years.

A referral from a Medicare provider (MD or DO) is required
